About MCCD
Medical Certificate of Cause of Death (MCCD) is the certificate issued by the attending medical practitioner who had treated the person during admission in a medical institution or in the last illness (prior to death) while taking treatment from a physician outside of a medical institution.
The cause of death is defined as "all those diseases, morbid conditions/abnormalities, injuries which either resulted in or contributed to death and the circumstances of the accident or violence which produced any such injuries". What can a participating centre do?

Strengthen the Medical Certification of Cause of Death (MCCD) through training
Record death events in a Registered Clinic or Nursing home or Hospital or Medical Institution
Record medical terms of cause of death, convert medical terms into International Classification of Diseases (ICD) codes
Record the underlying cause of death and the sequence of events leading to death
Quality checks to avoid mode of dying being referred as cause of death
Generate MCCD Form 4 and Death Report Form 2, necessary for death registration
Have restricted user access
Ensure data security and confidentiality
How is it helpful for the hospitals?
The following outputs can be generated:
- Level 1: Certificate of death for disposal of body: Death notification
- Level 2: Medical certification of cause of death (MCCD) - Form 4: Recording cause of death
- Level 3: Death report - Form 2: Submitted to Local Registrar's office by hospital along with Form 4

General physicians who certify death at home can also use the software to record and issue Form 4A
